Please check with your plan regarding co-pays, co-insurances, deductibles, and all out-of-pocket fees. Co-pays are due at the time of your visit. Payments can be made by cash, check, or major credit cards.
Due to the specialized nature of our practice, the majority of insurance plans will authorize consultation and operative procedures, even if the practice is not participating in the plan. Our office staff will work with you, your primary care provider, and your insurance company to obtain the necessary referrals and authorizations.
